The Community Dental Clinic serves to help bridge the gap for people who reside in Mercer County with no access to dental care.
Mission Statement
To improve the oral health of the community by addressing health disparities and inequities in dental treatment and education. The goal of the clinic includes identifying those residing in Mercer County who are in need of oral health care due to lack of financial resources and the inability to find a provider in a timely manner. Consideration will be given to all patients and factors to be considered will include the immediate nature of the problem I.e. pain swelling and function.
Our Focus
The Community Dental Clinic was established to bridge the gap in access to dental care in Mercer County. By offering a holistic approach to patient care, we seek to ensure that those in our community who are most vulnerable have access to essential oral health services. Our goal is to deliver not just treatment but also education, empowering patients to maintain their dental health long-term.

We offer a range of dental services to address common oral health needs, including:
- Comprehensive exams
- Preventive hygiene care
- Extractions
- Fillings
- Comprehensive Dental Exams
- Limited Oral Hygiene
- Restorative (including composites)
- Prosthetic restorations (fixed and removable including placement and restoration of implants on a case by case basis)
- Teeth extractions
- Oral pathology
- Limited Anesthesia

How to Schedule an Appointment
To ensure we serve those most in need, the scheduling process involves three steps:
- Referral from a Qualified Organization*: A referral form must be completed and submitted by a Qualified Organization on behalf of the patient. (Patients must reside in Mercer County.)
- Scheduling the Appointment: Once the referral is submitted, the patient can call to schedule the appointment directly. If the patient is unable to call, the Qualified Organization may schedule the appointment on their behalf.
- Submitting Patient Form: Must have an appointment scheduled prior to submitting patient forms. Before to the appointment, patients must complete their patient forms. Click the “Patient Forms” button below to access forms.
Who Can Submit a Referral?
To qualify for care, a patient must be referred by a Qualified Organization. These organizations meet specific criteria to ensure that individuals with the greatest needs are identified.
*Qualifying Organziation Criteria
- Works primarily with individuals facing financial, social, intellectual, or other significant hardships.
- Includes local dentists, non-profits, social service organizations, etc.
- Submits the necessary referral form and patient information on behalf of the individual.

How To Set Up An Account for Referrals
CREATE ACCOUNT
- Click on Referral Form Button (below)
- Sign Up (each organization only needs one account, multiple users can be added to the one account.)
- Select Dental Practice
- Create Primary Account and choose a user for the account
- Enter first & last name
- For role – select Dental Assistant, Hygienist, or Provider
- Enter Email and preferred password
- For Practice Information – enter your organization’s name and information
- Select Register
LINK WITH THE CLINIC
- Start On DDX Account Portal
- Go to My Partner dropdown
- Partner Type: Select Specialist
- Search Specialist Name: search for “Community Dental Clinic” and select
ADDING USERS TO YOUR ACCOUNT
- Click on Practice account
- Select the upper right hand dropdown
- Click Users
- Select Add New User
- Fill information like the steps above (choose same dental role)
You are now set up and ready to refer!
How To Submit A Referral
- Click on Referral Form Button (below)
- From Main Portal, click on New Task
- Choose New Referral
- On Specialist Provider dropdown – leave the default response “Jacob Pancake” selected
- Under patient details, leave Provider as is (this is the logged in user)
- Enter Patient name, email, gender, DOB, telephone
- In the Patient Chart field, if you do not have this data, enter “none”
- Procedures – reason you are referring. If you’re not sure select “Other – See Notes”
- Notes – This is the most important part to be able to share why you are referring and attach files (i.e. consent form if applicable)
- Submit Referral
- On the homepage of your account, you can see all referrals and status
