The Community Dental Clinic serves to help bridge the gap for people who reside in Mercer County with no access to dental care.
Mission Statement
To improve the oral health of the community by addressing health disparities and inequities in dental treatment and education. The goal of the clinic includes identifying those residing in Mercer County who are in need of oral health care due to lack of financial resources and the inability to find a provider in a timely manner. Consideration will be given to all patients and factors to be considered will include the immediate nature of the problem I.e. pain swelling and function.

The Community Dental Clinic was established to bridge the gap in access to dental care in Mercer County. By offering a holistic approach to patient care, we seek to ensure that those in our community who are most vulnerable have access to essential oral health services. Our goal is to deliver not just treatment but also education, empowering patients to maintain their dental health long-term.

We offer a range of dental services to address common oral health needs, including:
- Comprehensive exams
- Preventive hygiene care
- Extractions

- Comprehensive Dental Exams
- Limited Oral Hygiene
- Restorative (including composites)
- Prosthetic restorations (fixed and removable including placement and restoration of implants on a case by case basis)

How to Schedule an Appointment
To ensure we serve those most in need, the scheduling process involves three steps:
- Referral from a Qualified Organization*: A referral form must be completed and submitted by a Qualified Organization on behalf of the patient. (Patients must reside in Mercer County.)
- Scheduling the Appointment: Once the referral is submitted, the patient can call to schedule the appointment directly. If the patient is unable to call, the Qualified Organization may schedule the appointment on their behalf.
- Submitting Patient Form: Must have an appointment scheduled prior to submitting patient forms. Before to the appointment, patients must complete their patient forms. Click the “Patient Forms” button below to access forms.
Who Can Submit a Referral?
To qualify for care, a patient must be referred by a *Qualified Organization. These organizations meet specific criteria to ensure that individuals with the greatest needs are identified.
Qualifying Organization Criteria:
- Works primarily with individuals facing financial, social, intellectual, or other significant hardships.
- Includes local dentists, non-profits, social service organizations, etc.
- Submits the necessary referral form and patient information on behalf of the individual.
